The Direct Dental Savings Plan is not an insurance product, nor is it a product of a prepaid limited health service organization or a discount medical plan organization. Direct Dental Savings is a direct to consumer dental discount agreement provided only to patients of George A. Hoop, DDS, PA. Discounts listed under the Direct Dental Savings Plan are provided only on dental care performed in the office of George A. Hoop, D.D.S., P.A. and not any other dental office. Plan members are obligated to pay for certain dental services and will receive the appropriate discount at the time of service.

III) “Insurance”

The term “insurance” in this website or associated documents is used for comparison purposes only. The term “insurance” is not meant to imply that Direct Dental Savings is in any way an insurance product. Direct Dental Savings is not an insurance company nor is it a product of an insurance company.

IV) Required statement for compliance with F.S. 456.062:

THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, FOR TREATMENT.
